The Personalisation service helps you to make sure you are receiving the support that you need. We talk to you, and people who look after you, so that we can see what’s happening now, and then we will work with you to plan for the future.

We can help you with:

• Your supported self-assessment

• Your support plan

• A review of your current plan

• Making choices that are right for you

You will have a Personalisation Practitioner who you can talk to at any time whilst they are working with you, and who will make sure you are getting the support to meet your needs.

Case Study

John is 25 years old and has to use a wheelchair to get around. His mum and dad are the only people available to take him out, so often trips have had to be time limited. At weekends John likes to do social things. He has to have help with his personal care and “styling” for his trips out. Now that he has his new PA, John:

goes to a wide range of activities, including gym sessions, cinema and food shopping
