Title: Shared Lives Scheme Worker
Reports to: Shared Lives Deputy Manager
Accountable to: Shared Lives Team Manager
Location: Contracted Base – Lockwood Centre, Guildford. Post

requires travel across County

Hours: 37.5 hours per week

Job Purpose

As a Shared Lives Scheme Worker you will provide and deliver:

The post holder will be responsible for ensuring that the scheme delivers safe and person-centred care and support for vulnerable adults. The Shared Lives worker will be expected to oversee Shared Lives arrangements and provide support and monitoring to Shared Lives Carers. The scheme aims to promote individual independence and enable customers to achieve their goals. Therefore, the scheme worker will need to be able to work with customers with a range of needs in a personalised way to completed Shared Lives documentation.

Key Responsibilities

  • Support, monitor and review Shared Lives Carers
  • To manage Shared Lives duty system as needed
  • To process new referrals and match customers with Shared Lives Carers
  • To undertake Shared Lives carer assessments
  • Prepare and deliver training to prospective and existing Shared Lives Carers
  • Support the scheme with arranging and attending events to promote Shared Lives
  • Oversee arrangements and maintain paperwork in accordance with CQC Key Lines of Enquiry (KLOE)
  • Complete Customer related documentation such as; funding tools, support plans, reviews and risk assessments
  • Carry and manage a caseload in line with service delivery targets
  • Joint work with other professionals and teams as required in order to meet the needs of the customer
  • Be accountable for raising any concerns or safeguarding alerts immediately in line with the Surrey Safeguarding Adults Procedures

  • Take all reasonable precautions to maintain the health and safety of self and others, ensuring that any risks in relation to this are reported and remedial action taken in a timely manner
  • Take responsibility for own continuous professional development and maintaining an adequate level of knowledge and skills pertaining to the role and apply learning to practice
  • Undertake any other duties that may reasonable be required of this role and as requested by manager (s)

  • Good Grades in GCSE English and Maths or equivalent qualifications is essential
  • Care Certificate
Skills, Knowledge and Experience
  • Excellent Communication skills – verbal and written
  • Competent user of IT hardware and software
  • Ability to work independently as well as part of a team
  • Passion to provide high-quality person-centred services.
  • Good organisational skills. Ability to prioritise work in an environment which may have conflicting pressures and demands.
  • Knowledge, skill and working practice of undertaking assessments, support plans, reviews and risk assessments with attention to detail
  • Ability to work under pressure. Sound creative problem-solving skills and flexible approach to working with customers and Carers
  • Be self-motivated, proactive and able to work well with other in a multi-agency environment and under pressure, including responding in crisis situations.
  • Understanding of key relevant legislation and relevant of experience of operating within it
  • Access to a vehicle and willingness to use this as needed to transport customers and / or carers to / from visits.

Experience Required:

  • Experience of providing person centred care to people with Special Care needs
  • Experience of working within a team that provides social care
  • Experience of working with diverse communities and understanding of commitment to equal opportunities and anti-discriminatory practices

  • Able to manage own work load
  • Highly motivated to deliver person centred care planning
  • Ability to handle difficult situations and conversations whilst maintaining professional and customer centred standards
