- They have helped the people Surrey Choices support to find work experience or paid work

Being Collaborative Award
To recognise those who have worked together as partners to create better opportunities and stronger communities. These collaborations will have benefited the people we support, colleagues and the community.

Criteria
To be shortlisted for the award you must demonstrate the following:
- They have created new opportunities for the people Surrey Choices support in the community
- They have worked with key local people and groups who share the same values as Surrey Choices
- They have worked in partnership with others to implement change
- They have networked with key members of the community
- They demonstrate good joint working practices
- The partnership has enabled the people Surrey Choices support to develop
- They have promoted the partnership

Dream Team Award
To recognise a group of people or a team who have worked together to achieve positive outcomes for the people Surrey Choices support
Criteria
To be shortlisted for the award you must demonstrate the following:
- The group is of two or more people
- The group have demonstrated working together towards a shared outcome
- The group have achieved an exceptional level of customer service
- The group help to develop either their colleagues and team members or those Surrey Choices support
- The outcome of the group is achieved by a fair team effort
- The group work on projects to achieve a strategic outcome
- The group work with other services and teams within Surrey Choices
- The group works together to find solutions to everyday issues that may arise
- The group have high levels of effective communication

Being Courageous Award
To recognise colleagues for bravely trying new things, celebrating their achievements, and learning from mistakes.

Criteria

- That by being courageous they have improved the lives of the people that we support or that of their colleagues
- That they have evidenced and reflected on their work to improve and learn going forward
- That they have overcome barriers
- That they have used perseverance to make something happen
- That they have demonstrated originality in their approach
- They have used technology in their approach
- That they have shown willingness to put themselves forward for projects and contribute to the overall Surrey Choices Strategic Plan
- That they have shown commitment to their own personal development

To recognise those who aspire to be the best at what they do and empower each other to lead the way.

- That they have shown consistent reliability
- That they are demonstrating good practice in their work to enable others to achieve

To recognise those who value and show that they care for each other, families, colleagues, and communities.

Criteria

- That they support and care for others to make a difference to their lives
- That they value others and show them respect and dignity
- That they show the impact that their care and support has on others
- That they give examples of when and how the support others when they require extra support eg mental health crisis or behavioral support

- That they demonstrate how you have supported people to be active citizens in their communities
- That they demonstrate in their work how they respect everyone that they work in with clear examples of this good practice
- That they go above and beyond in their work to include everyone and putting them at the heart of what they do
- That they are demonstrating co-production where they enable the people we support and their colleagues to make choices and decisions together

To recognise those who encourage each other to speak up, and those who listen, take action and communicate well.

- That they can demonstrate in their work that they have truly listened to the people that Surrey Choices supports and their wants and wishes
- That they have given opportunities for people to voice opinions, concerns and enabled people to influence decisions
- That they are enabling the people Surrey Choices support to have all of the right information for them to make decisions
