Community Based Support
Tyler has been supported by Surrey Choices colleagues out in the community over the last couple of months. He has been working on his communication skills and becoming more independent whilst out and about.
Over the last few months Surrey Choices colleagues have learnt many new things about Tyler that they didn’t know before. They discovered that he loves walking around towns, castles and gardens and that he likes using the outdoor gyms in his local park.
Through Tyler’s Person Centred Plan, colleagues identified that Tyler wanted to explore more gym based activities and to take up swimming at his local leisure centre.
Heading out into the community has had a huge impact on Tyler. He is communicating more than he was before, and he has shown a willingness to become more independent with his decision making. He has also started working on his health and wellbeing which has been added to his Person Centred Plan.
Surrey Choices colleague Lottie Cowey said:
“Tyler had really come out of his shell since receiving community based support. We feel that his sensory needs and social interaction is being fulfilled greatly through community activities and respite. He is now making his own independent decisions during the day about what he wants to do and what he is having for lunch.”
