Person Centred Plans lead to independence in the community
Jacqueline loves to be out in the community and have independence to explore her local town.
Whilst she was out with Andrea Willson, Activity Facilitator, they spoke about some of her goals and Jacqueline said she wanted to swim at her local leisure centre.
Together they went swimming and Jacqueline loved every minute of it. From going down the big slides to swimming under water and playful splashing. Jacqueline said:
“I love it, I love it. Can we do it again!”
Working together on her person-centred plan has allowed Jacqueline to become more independent with the activities she wants to do and achieve in the community. This has allowed colleagues to see a different side to her.
“She is like a little mermaid. It is great to see her enjoying herself so much. It is now something she wants to do regularly and hopefully will start joining sessions that they provide at the Leisure Centre.” Andrea Willson.
